    Altar Server Application Form

    • Any parish boy who has completed his First Holy Communion and is of good faith is
      welcome to become an Altar Server. After completing an application, candidates
      undergo a period of formation before becoming a commissioned server. Candidates
      will receive appropriate training to get started and then will serve as Novices for a period of three months. During formation servers will learn about the Catholic
      Church and the Mass. Candidates will observe experienced servers and learn how to
      carry out specific tasks while serving at the altar.

      Parents must be willing to undertake an active role in supporting their child in this
      ministry by ensuring that their altar server arrives to their assigned Mass on time and
      properly dressed. In addition, Altar Server candidates must:

      • be an active member of the parish community or school,
      • believe in the teachings of The Holy Catholic Church,
      • have already made their First Confession and their First Holy Communion,
      • know how to make the sign of the Cross,
      • know how to receive Holy Communion in the proper manner,
      • be at least 10 years old.
